CASE MANAGEMENT FOR PERSONS WITH DISABILITIES (HOC)
Purpose:
To provide case management and rehabilitation services to low income individuals who are physically or mentally disabled and reside in housing funded by the Montgomery County Housing Opportunities Commission (HOC) with Stewart McKinney Homeless Assistance funds.
Individuals Served:
The program serves up to eight individuals with serious and persistent mental illness (SPMI) and/or other disabilities and up to ten families with a disabled head of household.
Program:
The program provides a master’s level social worker to provide case management services; 24-hour pager on-call back-up; development and maintenance of communication with funding and other relevant agencies; development and monitoring of clients’ service agreements and follow-up; development of individual treatment or rehabilitation plan for persons eligible for MHP services; and daily assistance and referrals to relevant agencies and other resources.

Hours of Operation:
Monday - Friday 9:00 AM - 5:00 PM Limited evening hours available.
Fees:
Grant funding covers these services for those who do not receive Mental Health Administrator (MHA) funded Psychiatric Rehabilitation Program (PRP) services.
Staffing:
One master’s level social worker and a case aide.
Location:
Services are provided throughout Montgomery County.
Referral Information:
Referrals come from the Montgomery County Housing Opportunity Commission.
