SANTÉ SENIOR SERVICES
Purpose:
To address the unmet mental health needs of Montgomery County homebound senior citizens age 60 and above.
Individuals Served:
- Older adults age 60 and above.
- Older adults age 60 and above who are homebound both physically and emotionally.
- Family caregivers of an older adult.
Program:
Sante Senior Services is a comprehensive Montgomery countywide initiative to address the unmet mental health needs of county seniors. Services are provided via four programs:
SORT-SENIOR OUTREACH TEAM
Senior Outreach team provides short-term therapy and geropsychiatric assessment to homebound seniors.
HISPANIC OUTREACH
Hispanic Outreach team provides bilingual short-term therapy to homebound seniors.
PREVENTATIVE MENTAL HEALTH SERVICES FOR SENIORS (PEMS)
Provide community outreach, drop in groups at designated senior centers, and consultation/collaboration to aging service professionals and community organizations.

Staffing:
Master’s Level social workers and bilingual counselor (Spanish) augmented by a psychiatrist when needed as well as administrative support staff.
Hours of Operation:
Monday - Friday 8:30 AM - 4:30 PM (Hours may vary according to client request)
