SPECIAL OUTREACH ASSESSMENT REHABILITATION (SOAR)
Purpose:
To assist individuals to improve life skills through goal-directed activities provided by one-on-one contact.
Individuals Served:
- Adults experiencing psychiatric, emotional, and behavioral disturbances, including those with a developmental disability
- Children over the age of 5 years who are experiencing psychiatric and/or emotional disturbances, including those with a developmental disability
Services:
Provides individuals with life-skills training. In addition, essential case management service is in place to ensure follow through and follow up throughout the process. The services of this outreach program are effectively provided with individuals, small groups within a home, school, community or, vocational setting.
Service focus may include:
- Life skills training
- Medication monitoring
- Case management
- Functional assessments
- Social skills support
- Health promotion and training
- Therapeutic recreation
Staffing:
Master’s and bachelor’s level rehabilitation specialists augmented by a multi-disciplinary clinical team.
Hours of Operation:
Monday - Friday 9:00 AM - 3:00 PM

- Montgomery County
- Prince George’s County
- St. Mary’s County
Fees:
Services must be pre-authorized by Maryland Medical Assistance (Medicaid) for payment
