PURPOSE   Resource Coordination services facilitate community-based services while matching individual needs.  Our goal is to assist individuals choose, get and keep services and support so that they can make informed choices and meet their goals. 

 

INDIVIDUALS SERVED

·                    Individuals between 18-65 years of age

·                    History of a brain injury

·                    Residents of Montgomery, Baltimore or Howard County

·                    Individuals not receiving service coordination or case management elsewhere

·                    Priority given to individuals who are financially needy

                           

RESOURCE COORDINATION SERVICES                     

Services are determined by individuals based on their needs and goals.  A description of each of these services follows.

 

Intake and Assessment

Functional and skills assessments that focus on individual needs, skills and goals.

 

Planning            

Identification of service preferences that results in planned rehabilitation path. 

 

Linkage

We work with our clients to identify and obtain community-based services and supports.

 

Monitoring

We continue to work with our clients after services and supports are identified and initiated to ensure progress towards goals established.

 

Advocacy

We advocate on behalf of our clients to ensure that their rights are respected and that services are developed or modified to meet their needs.
